Persistent Pain Can Feel Like Something Is Still Wrong — Even When Healing Has Occurred
If pain always accurately reflected injury, understanding the body would be much simpler. A minor injury would cause mild pain, a serious injury would always cause severe pain, and every scan would perfectly explain what someone was feeling.
But persistent pain does not always work that way.
Pain is a real protective experience produced by the nervous system, and its intensity does not always match the amount of tissue damage present. This helps explain why some people experience significant persistent pain after an injury has healed, while others may have structural changes on a scan without experiencing pain at all.
Australian health guidance describes persistent or chronic pain as pain lasting more than three months or beyond the expected healing time. It can continue even when there is no ongoing damage to the body.

In clinical practice, David regularly meets people with musculoskeletal injuries, headaches, sporting conditions and persistent pain who are understandably worried about what their symptoms might mean.
Many people assume that the more something hurts, the more damage must be occurring. When that belief leads to fear of bending, lifting, exercising or returning to everyday activities, it can make rehabilitation more difficult.
Understanding persistent pain can help people recognise that pain is important, but it is not always a precise measure of tissue damage.
What Is Persistent Pain?
Persistent pain is pain that continues beyond the usual healing time, often for more than three months. It is also commonly referred to as chronic pain.
In acute pain, such as immediately after an injury, pain often acts as a useful warning signal. It encourages you to protect an injured area while tissues recover.
With persistent pain, that relationship can become more complex. The original injury may have healed, yet the nervous system may remain more sensitive and continue producing a protective pain response.
This does not mean the pain is imagined. Persistent pain is real, even when there is no clear ongoing tissue damage to explain its intensity.
Why Persistent Pain Doesn’t Always Equal Damage
A helpful way to think about pain is as an alarm system rather than a damage meter.
Your nervous system constantly receives information from your body and surroundings. It processes signals from muscles, joints and nerves, but it is also influenced by factors such as sleep, stress, emotions, previous experiences and the situation you are in.
The brain and nervous system use all of this information when determining whether protection is needed.
With persistent pain, the alarm system can sometimes become more sensitive. Queensland Health describes this using the comparison of an overly sensitive car alarm that can be triggered by something relatively minor.
That means a movement may feel painful even when it is not causing further tissue damage.
Persistent Pain and Scan Findings Don’t Always Match
Scans such as X-rays and MRIs can be very useful when they are clinically indicated, but they are only one part of understanding pain.
Structural changes such as disc bulges, joint degeneration or tendon changes may sometimes appear on imaging in people who have little or no pain. At the same time, another person may experience significant persistent pain without imaging showing an equivalent level of structural change.
This does not mean scan findings should be ignored. It means they need to be interpreted alongside your symptoms, medical history, physical examination and overall clinical picture.
When assessing persistent pain, the question is not simply, “What does the scan show?” It is also, “How is this person functioning, what aggravates their symptoms, and what other factors may be influencing their pain?”
Why Can Persistent Pain Continue After an Injury Heals?
After an injury, the nervous system can temporarily become more sensitive. This is useful because it encourages you to protect the area while healing takes place.
For some people, however, this sensitivity can continue.
Healthdirect explains that in chronic or persistent pain, nerves may become more sensitive than usual, and the brain may process signals differently.
Several factors may influence this increased sensitivity, including:
- previous injury
- reduced sleep
- stress
- illness
Not every person with persistent pain will experience the same combination of factors. Pain is highly individual, which is why management needs to consider more than just the original injury.
Why Gradual Progression Matters for Tissue Capacity
Building tissue capacity is not about pushing through as much exercise as possible.
Too large an increase in workload may aggravate symptoms or exceed what an injured area can currently tolerate. On the other hand, consistently doing less than your body is capable of may not provide enough challenge to achieve your rehabilitation goals.
The appropriate middle ground is individual. Progressive rehabilitation increases the challenge as your tolerance improves rather than relying on a fixed amount of exercise for everyone.
Some discomfort during rehabilitation can be acceptable for certain conditions, while other presentations require greater caution or temporary protection. Your diagnosis, symptoms and response to activity all matter when deciding how quickly to progress.
Why Understanding Persistent Pain Matters
When people believe that every painful sensation means damage is occurring, avoiding movement can feel like the safest option.
Someone living with persistent pain may begin avoiding exercise, bending, lifting, running or even everyday activities because they are worried about making the problem worse.
Short-term protection can be appropriate after some injuries. However, prolonged avoidance may contribute to reduced strength, fitness, mobility and confidence.
Over time, this can create a cycle where activity decreases, physical capacity drops and movement begins to feel even more difficult.
Understanding persistent pain can help separate the experience of pain from the assumption that every painful movement is harmful.
How Can Persistent Pain Affect Confidence in Movement?
One of the biggest challenges with persistent pain is that people can gradually lose trust in their bodies.
A movement that once felt normal may begin to feel threatening. If that movement causes pain, it is understandable to want to avoid it.
However, avoiding every painful or uncomfortable movement is not always the most helpful long-term strategy. In many persistent pain presentations, rehabilitation involves gradually rebuilding confidence and physical capacity rather than waiting until all pain disappears before moving again.
The goal is not to ignore persistent pain. It is to understand what the pain may mean and find an appropriate level of activity for the individual.
Persistent Pain Should Never Be Dismissed
Understanding pain science should never be used to tell someone that their symptoms are “all in their head”.
Persistent pain is real, and it can have significant physical, emotional and social effects. Queensland Health notes that persistent pain can affect people physically, emotionally, psychologically and socially, while those same factors can also influence the pain experience.
The fact that persistent pain may continue without ongoing tissue damage does not make it less important.
Instead, it tells us that pain needs to be considered as part of a broader picture rather than viewed only as evidence of structural damage.
When Persistent Pain Needs Further Assessment
Although persistent pain does not always mean that tissues are continuing to be damaged, new or changing symptoms still deserve appropriate assessment.
Pain following a significant injury, sudden severe pain or symptoms accompanied by concerning neurological or systemic changes should not simply be attributed to nervous system sensitivity.
